By registering my child, I am committing to the concert dates. It is expected that my child will participate in the concerts. If there is a scheduling conflict, I must communicate with the choir director at least one month prior to the concert. Obviously, if there is a conflict that arises, I will communicate with the choir director ASAP.
I understand that tuition is due by two weeks after rehearsal start date and a late charge applies to tuition payments after this date ($20).
I understand that the entire tuition is nonrefundable as of two weeks after rehearsal start date. I further understand that I must communicate to the director of DYC before this date if my child will not be continuing the program and I want a refund.
I understand that if my child is unable to meet behavior rules in the classroom, they can be dismissed from the group at the discretion of the Director, and that the tuition is non-refundable as of two weeks after rehearsal start date.
I understand that parent communication to the staff, other DYC parents and children must be respectful whether by email, text, phone, or in person. Parents are expected to defer to DYC staff regarding behavioral or organizational decisions. I understand that my child must take direction from DYC staff. If parent conduct or communication violates these codes, my family may be suspended or dismissed from DYC at the discretion of the director. Tuition is non-refundable as of two weeks after rehearsal start date.
I will provide a “safe list” of people who are authorized to pick up my child, and I will update DYC staff as needed.
If my child can bicycle or walk home, I will provide appropriate permission to DYC in writing.
Concert “stoles” will be handed out at the concerts, and must be returned immediately afterward. If the stole is not returned, I will pay $20 to DYC for a replacement.
If my child exhibits symptoms of illness (cough, stuffy or runny nose, fever), my child will not attend rehearsal. If my child displays these symptoms during rehearsal, I understand I will be called by DYC staff and I will pick up my child immediately.
I will provide DYC staff information for any special needs my child has. Staff will try to accommodate the special needs, but the child/family should have a plan in place if the needs cannot be accommodated.
I understand DYC is a group activity and absences degrade the quality of the group. I commit to making DYC rehearsals a priority.
I give DYC permission to photograph and videorecord my child during rehearsals, breaks and concerts. I understand these pictures and videos may be posted to DYC’s website; the “Big Day of Giving” website; and social media as pertains to DYC operations, publicity, and fundraising drives.
